Why work in home health care or hospice?

Our Home Health Care patients are homebound. Our clinical staff make visits to patients in their environment, working with them and their caregivers toward continued progress to recover from an illness, accident or injury. Without the Home Health Care clinician, fragile patients would be required to arrange other means of getting the care they need; often with a great deal of taxing effort.

Our Hospice patients benefit from the ability to stay in their home, surrounded by their loved ones and friends thru the process of living with a life threatening illness. Our Hospice team provide skilled care, medication management, and support for the patient and family.


Required skills for home health care and hospice clinicians

Home Health Care, or Homecare, is a comprehensive array of health services provided to people at home for the purpose of promoting, maintaining or restoring health or minimizing the effects of illness and disability in order to achieve and maintain the highest level of health, function and independence.

Required Skills for Home Health Care Visit Staff

  • Flexible and Adaptable
  • Compassionate and respectful of people in their own homes
  • Critical Thinking skills and independently led
  • Effective Communicator, both verbally and in writing
  • Proficient with computers and familiarity with electronic medical records (EMR’s)
  • Homecare Homebase Experience a plus but not required.
  • Ability to give and take direction and be an effective team member
  • Ability to manage and prioritize multiple tasks while maintaining stellar attention to detail
  • Ability to identify the most important needs of the patient and provide effective and efficient care in a complex situation amidst the distractions of a home environment
  • Ability to accept patient’s social and clinical challenges without judgment
  • Ability to use a variety of tools to advise and lead patients to improved self-care and effective disease management
  • Owns and maintains a reliable method of transportation for all seasons.

Covid-19 Pandemic

Central New Hampshire VNA & Hospice continues to serve patients and will plan to do so into the foreseeable future. Working closely with our community partners, our goal during this pandemic is to provide the needed home based services our community requires.
It is our intention that our patients will not see a big change in their care. What they might notice is the addition of a number of screening questions when scheduling a visit with one of our clinicians.
