Central New Hampshire VNA & Hospice strives to improve the health and development of children and families through our Pediatric Programs by providing prenatal visits, mother-baby postpartum visits, pediatric home nursing care, and pediatric care management. Using a family-centered approach, we support and teach families through a variety of services and programs.

Pediatric Home Visits

Visits made by a licensed registered nurse, for children and adolescents with acute or chronic illness or injuries.

Prenatal Home Visits

Visits made by a licensed registered nurse, helping pregnant women who are Medicaid-eligible. Nursing visits will include prenatal guidance, parenting skills, education, information, resource referrals.

Mother-Baby Postpartum Visits

Visits made by a licensed registered nurse, helping to get new parents and their babies off to a great start. Our nurses can help in the teaching of child development stages and in supporting good parenting skills.

Home Visiting New Hampshire

In collaboration with Community Action Partnership, Home Visiting NH offers Registered Nurse and parent educator home visits for pregnant women who are Medicaid-eligible. Our staff works with families to connect them with medical care, provide breastfeeding education and guidance, monitor infant growth & development, provide parenting education and support and referrals to other resources & supports related to the family’s need (WIC nutrition program, food stamps and house assistance). Our goal is to make sure clients receive the care they need to nurture a healthy and secure baby.

Healthy Families America

All expectant parents and parents of newborns have common questions about their child’s development. Healthy Families America is a nationally recognized, voluntary service that connects overburdened expectant parents and parents of newborns with free child development assistance in their homes. Based upon years of evidence, we know that home visiting promotes optimal long-term mental and physical health of parents and their children.
