Healthcare at Home

To ensure the best possible outcomes for our patients, our dedicated team of home healthcare professionals work with your physicians to understand your medical needs.

As a patient focused team approach, we are able to work with you and your caregivers to help you achieve your specific goals toward recovery post-surgery, illness or accident. No need to leave your home for treatment. For qualified patients; we are able to bring skilled nursing, licensed nursing assistants, therapy and medical social services to you, in the comfort of your home. Ask your physician or simply contact us directly if we can help in providing comfort and independence to you or a family member.

When is healthcare at home appropriate?

A patient or physician may request home healthcare services
when one needs support:

  • Recovering from illness, accidents or surgery
  • Managing a new medical diagnosis
  • Coping with a chronic illnesses
  • Managing complex medications

Corona Virus or COVID-19 Pandemic:

Central New Hampshire VNA & Hospice continues to serve patients and will plan to do so into the foreseeable future. Working closely with our community partners, our goal during this pandemic is to provide the needed home based services our community requires. It is our intention that our patients will not see a big change in their care. What they might notice is the addition of a number of screening questions when scheduling a visit with one of our clinicians.
